Care Coordinator/Caseworker

Mino Care is seeking a compassionate and motivated, Care Coordinator/Caseworker to join our team and perform duties in line with our vision, mission, and goals. The Care Coordinator/Caseworker ensures that clients receive comprehensive and coordinated care, collaborates with clients and families to provide counselling, and identifies suitable support for their needs. Communication, problem-solving, and organisational skills are essential for success in this role.

Roles/Responsibilities (include, but are not limited to):

• Conducts comprehensive assessments, and follow-up calls with clients and is experienced and comfortable making phone calls

• Provides counselling and ongoing support to clients, identifying needs and facilitating access to services

• Maintains accurate records, collects case management statistics on a daily basis and prepares an outcome measures report on a monthly basis to the Program Coordinator

• Remains informed about available community resources and support

• Performs other related duties incidental to the work described herein

Qualifications

• Bachelor’s or Master’s degree in Social Services, Social Work, Psychology, Public Health, Nursing or related field, with one-two years of case management experience. A combination of education and experience may be considered

• Possesses First Aid and CPR certifications

• Demonstrates knowledge of community-based resources and maintains confidentiality and ethical guidelines

• Ability to assess needs or risks and experience with crisis-intervention methodologies

• Excellent communication, active listening and interpersonal skills

• Excellent organizational skills with the ability to multitask to manage multiple cases and can work independenty

• Strong problem-solving and analytical skills

• Empathy and compassion towards clients

• Demonstrates cultural competency and sensitivity

• Computer literacy in G-Suite is necessary

• Ability to undergo applicable screening and background checks

Time Commitment: 15 hours per week on a part-time basis (Tuesdays, Wednesdays, and Thursdays with daytime availability-no evenings)

Pay: $30/hr
