Care Partnerships Manager

Mino Care – Perinatal Pathways Program $57,600 annually ($40/hour), 30 hours/week Location: Hybrid (Toronto/GTA preferred) Start Date: Feb 01, 2026.

Mino Care is offering a Perinatal Pathways Program, which is a multilateral, cross-institutional referral model that connects hospitals, midwifery clinics, community health centres, private clinics, social service agencies, and Black-led community organizations into a coordinated ecosystem of care.

Position Overview

The Care Partnerships Manager will lead the development, coordination, and maintenance of institutional partnerships across the perinatal, reproductive, and social services landscape. This role is ideal for someone experienced in inter-organizational relationship building, health system navigation, and information systems at provincial and federal levels, who is passionate about equity, collaboration, and culturally grounded care.

You will support the expansion and formalization of referral agreements, oversee operational integration of Mino Care staff within partner institutions, and ensure smooth cross-sector collaboration aligned with the three pillars of the Perinatal Pathways Program.

Roles/Responsibilities (include, but are not limited to):

Partnership Development & Management

· Establish, formalize, and sustain referral partnerships with hospitals, midwifery clinics, community health centres, private clinics, CPNP programs, and social service agencies.

· Lead the design and negotiation of partnership agreements, MOUs, and system workflows.

· Represent Mino Care in cross-organizational meetings, committees, and joint planning sessions.

Program Integration & Coordination

· Facilitate the operational onboarding of Mino Care’s onsite staff teams within partner sites.

· Support the implementation of culturally safe care pathways and navigation processes.

· Identify opportunities for joint training, reciprocal learning, and shared accountability.

System Navigation & Information Systems

· Apply in-depth knowledge of provincial and federal health systems, referral pathways, and data governance frameworks.

· Collaborate with the Care Information Manager to ensure alignment between partnership workflows and information-sharing structures.

Quality Improvement & Reporting

· Monitor partnership performance, referral volumes, and operational metrics.

· Contribute to shared outcome reporting and cross-institutional quality improvement.

Qualifications

· 3–5+ years of experience in partnership management, health system coordination, or institutional relationship development.

· Strong understanding of provincial and federal health systems, referral models, and health information systems.

· Experience working with (or within) hospitals, midwifery practices, community health centres, or social service organizations.

· Demonstrated commitment to culturally safe, equity-centered practice.

· Excellent communication, negotiation, and collaborative leadership skills.

30 hours/week

$57,600 annually ($40/hour)
